ABSTRACT
TO study the visual outcome both far & near vision, spectacle independence and subjective symptoms of glare & halos in patients with manual small incision cataract surgery with MFIOL
Method: A prospective study was conducted on 100 eyes of 70 patients who under went manual SICS with MFIOL (diffractive type).
The patient were evaluated post operatively on 1st day, 1 week, 2nd week, 1 month, 2 month, 3rd month for distant vision , near vision , subjective symptoms of halos and glare
Result: 80% of patient had distant visual acuity 6/6, 20% had 6/9 .75% had near vision of N 6, 20% had N8, 5% N12.70% of the patient were spectacle independent. 10% of patients had glare & halos
Conclusion Patient with multifocal IOL experienced reduced spectacle dependence with good satisfaction despite some reports of halos. Hence manual SICS with MFIOL is an effective alternative to phacoemulsification with multifocal IOL in developing world with good visual outcome

INTRODUCTION
Although phacoemulscification is the preferred cataract surgical technique worldwide, a less expensive method is gaining popularity in the developing world where a growing backlog of cataract blindness has resulted from insufficient health-care resources. 1-3 With sutureless manual small-incision cataract surgery (SICS), the undivided nucleus is extracted and a intraocular lens (IOL) is implanted through a selfsealing scleral pocket incision. This technique is less expensive in terms of capital equipment investment, equipment maintenance, and disposable costs per case, less time consuming, less surgical complications, machine independent, small learning curve. In addition, it may be faster and better suited for the advanced and mature cataracts 4-5.
Cataract surgery has evolved over the years, at present the emphasis is on nature of IOL being implanted. Different designs were introduced over period of time. But loss of accommodation and dependence on spectacles for near work was frustrating. Multifocal IOLS raises a new hope for the patients. Phacoemulsification with MFIOLS surgery benefits the patient to the extent of attaining distant and near vision with out glasses and least astigmatism, but this machine dependent procedure is very costly and all patients cannot afford it specially in developing country like INDIA.
