Abstract

The purpose of the grant proposal was to collaborate with a host agency to establish a new multi-facet program designed for non-minor dependents placed in a Transitional Housing Placement Plus Foster Care (THP+FC) in Los Angeles County. A literature review was conducted to examine risk factors for youth transitioning out of foster care, the systemic barriers foster youth encounter, and the implementation of supportive service that can improve outcomes for youth aging out of foster care. The prevention and aftercare program will be implemented at Olive Crest in Los Angeles County and will provide supportive services. The goal of this program is to provide non-minor dependents in THP+FC with the necessary skill sets and support instrumental in their transition to independence. Submission or funding of this grant was not a requirement for the completion of the project.
