Abstract

Condylar hyperplasia is a condition that affects not only the proportions and facial symmetry in patients, but also static and dynamic occlusion functions with repercussions in the masticatory activity, the health of the temporomandibular joint (TMJ), and the anatomy and volume of adjacent soft tissues. Therefore, according to its severity this disease concerns maxillofacial surgeons, orthodontists, physical therapists, plastic surgeons, and nuclear doctors, who are all closely involved in the diagnosis stage. Historically, diagnosis of condylar hyperplasia has been based on anamnesis and the initial physical examination of the patient, where asymmetry, malocclusion and in some cases temporomandibular disorders (TMDs) are detected and later confirmed with tests such as bone scan and eventually by pathology report once condylar surgery has been done. The purpose of this literature review is to provide detailed information on the behavior of this disease from the point of view of its etiology, clinical characteristics, and distribution by age, sex and affected condyle, as well as the necessary diagnostic and imaging aids for its diagnosis, differential diagnosis, associated diseases, histological characteristics of the affected tissues, and the different therapeutic approaches according to severity, patient's age, and active or inactive form of the condition. The information was obtained from scientific research articles in different journals and literature reviews, taken from databases such as MEDLINE, EMBASE, and PubMed.

RESUMEN. La hiperplasia condilar es una patología que no solo afecta las proporciones y la simetría facial en los pacientes, sino que también afecta la función estática y dinámica de la oclusión con repercusiones en la actividad masticatoria, la salud de la articulación temporomandibular (ATM) y la anatomía y volumen de los tejidos blandos adyacentes. Por lo tanto, es una entidad que, según su severidad, compete a cirujanos maxilofaciales, ortodoncistas, fisioterapeutas, cirujanos plásticos y médicos nucleares, quienes están muy relacionados en la etapa de diagnóstico. Históricamente, el diagnóstico de hiperplasia condilar se ha basado en la anamnesis y el examen físico inicial del paciente, en donde se detectan la asimetría, la maloclusión y en algunos casos desórdenes temporomandibulares (DTM), que luego son corroborados con exámenes como la gammagrafía ósea y, finalmente, por el informe de patología después de que la cirugía condilar se ha realizado. El propósito de esta revisión bibliográfica, es conocer de manera detallada el comportamiento de esta patología desde el punto de vista de su etiología, sus características clínicas, su distribución por edad, sexo y cóndilo afectado, así como las ayudas diagnósticas e imagenológicas necesarias para su diagnóstico, las enfermedades asociadas y su diagnóstico diferencial, las características histológicas del tejido afectado y sus diferentes abordajes terapéuticos según la severidad, la edad del paciente y la patología en su forma activa o inactiva. La información se obtuvo de artículos de investigación científica, publicados en diferentes revistas y revisiones de la literatura, tomados de bases de datos como MEDLlNE, EMBASE y PubMed.

INTRODUCCIÓN

La Hiperplasia Condilar (HC) es una condición patológica autolimitante que genera deformidad facial severa a expensas de asimetrías mandibulares. Se caracteriza por un crecimiento exagerado y progresivo del cóndilo y puede comprometer cuello, rama y cuerpo mandibular, tiende a ser unilateral, aunque existen condiciones bilaterales y puede estar acompañada de dolor y disfunción articular.1 Es comúnmente encontrada en pacientes entre los 11 y 25 años de edad en su forma activa y, después de esta edad, encontrarse en su forma pasiva, más como la secuela clínica que dejó la enfermedad.1, 2 Aunque también se puede presentar en su forma activa en pacientes entre la tercera y quinta década de vida, e incluso antes de los diez años de edad y se ha reportado una mayor prevalencia en las mujeres que en los hombres.3 La patología se puede manifestar de tres formas: Hiperplasia Hemimandibular (HH) que corresponde a la asimetría por crecimiento en sentido vertical, Elongación Hemimandibular (EH), correspondiente a la asimetría por un crecimiento en sentido horizontal y una forma híbrida entre estos dos tipos.4

Su etiología se debe a factores que pueden ser genéticos, traumáticos, funcionales, tumorales y hormonales.4, 5

La impresión diagnóstica de la HC puede ser hecha por una combinación de hallazgos clínicos y radiológicos, posteriormente corroborada por exámenes de medicina nuclear como la gammagrafía y, por último, confirmada por patología cuando el paciente es sometido a intervención quirúrgica.

Existen diversas ayudas diagnósticas que van desde fotografías extraorales, modelos de estudio estáticos y articulados, radiografía panorámica, radiografía posteroanterior, tomografía con reconstrucción 3D, gammagrafias óseas planares y tipo SPECT y estudios histopatológicos para el diagnóstico de la hiperplasia.6

Su diagnóstico temprano es importante debido a que las modalidades del tratamiento difieren considerablemente, de acuerdo con las estructuras afectadas, la edad del paciente, la severidad de la asimetría y con el estado activo o pasivo de la patología.

Es necesario tratarla y definir si se elimina el centro del crecimiento en casos en los que se demuestra la hiperplasia condilar activa, o, por el contrario, tratar la secuela con cirugía ortognática y/o compensación dentoalveolar ortodóncica cuando la patología está inactiva. Es por esto importante un adecuado diagnóstico de la patología, y es allí donde la gammagrafía ósea de ATM debería ser esencial en la entrega de información adecuada al clínico, idealmente mediante datos cuantitativos.7

DEFINICIÓN

La HC es una alteración caracterizada por crecimiento excesivo y progresivo, que afecta la cabeza y cuello del cóndilo, el cuerpo y las ramas mandibulares. Es una enfermedad autolimitante y deformante, porque el crecimiento es desproporcionado desde antes de terminar el crecimiento general del individuo, y podría continuar hasta más allá de su finalización.4 Es una patología que afecta no solo la estética facial del paciente, sino la función a nivel masticatorio. Es una entidad que compete no solo a los cirujanos maxilofaciales sino a los ortodoncistas que buscan llevar al paciente a una oclusión normal, la cual se ve directamente afectada por una patología de base, como lo es una hiperactividad celular a nivel condilar.1

CLASIFICACIÓN

Obwegeser y Makek4 clasificaron la asimetría asociada a HC en tres categorías: Hiperplasia Hemimandibular (HH), asimetría por crecimiento en sentido vertical, Elongación Hemimandibular (EH), asimetría por un crecimiento en sentido horizontal y una combinación de estos dos tipos.

La HH se caracteriza por una ampliación tridimensional de un lado de la mandíbula, con un crecimiento excesivo en el cóndilo, donde el cuello condilar se observa delgado y la altura de la rama es mayor. Se puede encontrar una mordida abierta ipsilateral o sobre erupción de los dientes maxilares buscando lograr la oclusión, hay muy poca desviación del mentón y se presenta asimetría con descenso de la comisura del lado afectado. Existe una compensación en el tercio medio por alargamiento del proceso alveolar, así como una inclinación hacia abajo del plano oclusal en el lado afectado e incluso desnivel en el plano orbitario, pero las líneas medias dentarias generalmente coinciden.4

La HE es el tipo más común de HC, se caracteriza por una cabeza condilar en forma y tamaño normal, pero acompañada de un cuello elongado. Existe una desviación del mentón hacia el lado contralateral, pero no hay aumento ver tical de la rama. El ángulo mandibular presenta menos alteración que cuando hay crecimiento vertical. Intraoralmente, la línea media se desvía hacia el lado no afectado y en la oclusión se observa, generalmente, mordida cruzada contralateral, mientras el lado afectado genera desplazamiento en sentido mesial hacia una clase III de Angle.4

El borde inferior se mantiene sensiblemente igual y los cambios compensatorios del tercio medio son ligeros, aunque puede haber un leve desnivel que se manifieste hasta el piso de la órbita, además, es posible que el filtrum del labio superior así como la punta nasal se inclinen hacia el lado no afectado.

Por otro lado, cuando la elongación es bilateral se presenta un severo prognatismo mandibular con o sin asimetría.

La tipo III es la forma híbrida. En esta se desarrollan la elongación y la hiperplasia en el mismo lado. El lado afectado es más voluminoso, con su borde inferior situado más abajo que el del lado contrario. Hay prominencia del mentón con desplazamiento hacia el lado no afectado y una marcada inclinación del plano oclusal.8

ETIOLOGÍA

La etiología es controvertida, sin embargo, se destacan el factor genético (malformaciones congénitas, microsomía hemifacial, atrofia hemifacial), factores ambientales (trauma prenatal y potsnatal, infecciones, deficiencias en el aporte sanguíneo e hipervascularización, así como trastornos neurotróficos), factores funcionales (interferencias oclusales y hábitos), factores hormonales (problemas endocrinos y factores de crecimiento), y factores neoplásicos (osteoma, osteocondroma y condroma).7, 9, 10

INCIDENCIA

La HC se presenta generalmente unilateral y se ha reportado en la literatura una significativa predisposición de las mujeres comparado con hombres (razón 2:1), con una prevalencia para el cóndilo del lado derecho de 57% respecto al cóndilo del lado izquierdo de 43%.3 Raijmakers y colaboradores,3 en 2012, repor tan que pacientes mujeres con HC unilateral superan en número a los pacientes masculinos en las poblaciones de estudio, por lo que el género femenino puede ser considerado un factor de riesgo para la HC Unilateral (HCU).

Se ha relacionado la predilección femenina con las diferencias hormonales, especialmente de estrógenos, ya que la mayoría de los pacientes reportados en la literatura con HC se han encontrado en la etapa de desarrollo de sus características sexuales secundarias, entonces es posible que las hormonas sexuales estén involucradas en el crecimiento anormal de la mandíbula.3 La explicación puede estar en que los estrógenos son uno de los principales reguladores de crecimiento de los huesos, y se expresan en cartílago articular y placas de crecimiento, ya que se ha encontrado evidencia experimental de síntesis local de estrógenos en la articulación temporomandibular.10, 11

CARACTERÍSTICAS

Fue descrita por primera vez por Robert Adams, en 18366, como una condición que hace que el desarrollo excesivo de la mandíbula genere alteraciones estéticas y funcionales. Desde entonces, ha habido numerosos escritos en la literatura refiriéndose a esta entidad clínica y el compromiso que produce en la estética facial y en la alteración ocluso-funcional.

Dentro de las características de la HC, se incluyen agrandamiento del cóndilo mandibular, unilateral o bilateral, en el que el cuello del cóndilo se agranda hacia afuera, inclinándose y con un crecimiento hacia abajo del cuerpo y de la rama de la mandíbula, que causa agrandamiento de ese lado de la cara y aplanamiento del lado contralateral. Algunos pacientes pueden presentar síntomas de la articulación temporomandibular (DTM), tales como dolor, sonido de la articulación y limitación de la apertura de la boca.12 El paciente entonces consulta por una franca asimetría facial con desviación mandibular, maloclusión y, en algunos casos, sintomatología articular, que se detecta usualmente entre la segunda y la tercera década de vida.4, 12

Generalmente se produce en la adolescencia hasta el cese del crecimiento puberal. Si la deformidad ocurre antes de que el crecimiento se haya completado, el plano oclusal generalmente presenta una inclinación por compensación dental, mientras que una mordida abierta posterior puede ser evidente si la deformidad ocurre después de que se ha completado el crecimiento.13

Algunos fenómenos biomecánicos podrían explicar por qué el crecimiento deformante se presenta en la rama y el cuerpo mandibular. Teniendo en cuenta los mecanismos que mantienen las fuerzas de compresión y tensión funcionales, dentro de un rango que es compatible con la salud del tejido, si se presentan situaciones que produzcan cargas que exceden la capacidad funcional de los tejidos ar ticulares como el trauma, la parafunción o el crecimiento excesivo y rápido, los vectores de fuerza producirían un cambio espacial de las estructuras del sistema, generando una secuencia de eventos desde la remodelación hasta la degeneración y reparación, resultandoen unaredistribución defuerzas.Estoscambiosen la articulación son seguidos por cambios morfológicos en la rama ascendente ipsilateral y el cuerpo mandibular con repercusiones en la armonía facial.4

HISTOPATOLOGÍA DE LA HC

La hiperplasia se conoce como el aumento en el número de células en un tejido, ocasionando en este caso un desarrollo excesivo del cóndilo. Histológicamente el estado activo de la HC se caracteriza por la presencia de una capa de células mesenquimales indiferenciadas y una capa de cartílago hiper trófico, con presencia de islas de condrocitos en el hueso trabecular que captan el tecnecio en la gammagrafía y su tasa de calcificación pareciera estar por encima de los rangos de normalidad.12 Este cartílago es muy grueso y se reemplaza por hueso nuevo. Esto se traduce en hipermetabolismo en el centro de crecimiento condilar del lado afectado, con conversión del cartílago hialino en fibrocar tílago.2

De acuerdo con Gray y colaboradores,12 en 1990, hay otro tipo de hiperplasia en la tercera y cuar ta década, que se caracteriza por una reacción tardía de crecimiento, en que histológicamente se observan grandes masas de cartílago hialino asociada a áreas degenerativas en la ATM con sintomatología dolorosa.6, 9 El centro de crecimiento, sin embargo, se localiza en el centro del cóndilo y no del fibrocar tílago. Cuando cesa el crecimiento, el aspecto histológico es de un cóndilo normal con una forma irregular.1

Con base en criterios histopatológicos y clínicos, la HC se ha clasificado en dos tipos: HC Activa e HC Inactiva (situación estable en el cual el crecimiento anormal se ha completado).14, 15 Si corresponde a un estado activo, exhibe una superficie articular continua y una amplia zona de proliferación en la que la capa de car tílago hipertrófico muestra un espesor medio. El estado inactivo se caracteriza por una superficie articular discontinua y una zona de proliferación a parches, la capa hipertrófica presenta un espesor variable y un gran número de condrocitos hipertróficos agrupados en algunas áreas.16

DIAGNÓSTICO

El diagnóstico de las asimetrías craneofaciales, dentro de las cuales se encuentra la HC, es importante debido a que las modalidades del tratamiento difieren conside - rablemente de acuerdo con el origen de la deformidad, es decir, si es de origen dental, esquelético o funcional. Existen métodos que han sido utilizados para identificar y cuantificar la magnitud de las asimetrías faciales y dentales, entre los cuales se incluyen la anamnesis, la evaluación clínica directa, los análisis radiográficos y la evaluación de las proporciones verticales y horizontales de la cara a partir de fotografías faciales.7

Muchas ayudas diagnósticas se han empleado para complementar el examen clínico y físico de esta patología, como lo son radiografías extraorales, análisis de fotografías, especialmente fotografías de frente, verificando una perfecta posición de la cabeza para evitar alteraciones en el estudio de las proporciones. Los modelos de estudio también son de gran ayuda en el estudio de las asimetrías faciales, ya que están acompañadas de un componente dental.17

MÉTODOS RADIOGRÁFICOS PARA EL DIAGNÓSTICO DE HC

Las radiografías convencionales se utilizan para determinar la localización y las discrepancias uní o bilaterales que contribuyen a la asimetría esquelética, como son el alargamiento del cóndilo mandibular/cuello del cóndilo involucrado y la inclinación del cuerpo y rama mandibular,13 así como para evaluar el grado de desviación de líneas medias dentales y esqueléticas desde el plano medio sagital. Adicionalmente, por medicina nuclear se tiene la Gammagrafía Ósea Planar y el SPECT (Tomografía Computarizada por Emisión individual de Fotones), que permiten identificar claramente un proceso de crecimiento condilar activo entre zonas inactivas.

Radiografía lateral de cráneo: da información útil sobre asimetrías ver ticales, la proyección sagital de la asimetría en altura de ramas, longitud mandibular y ángulo goníaco. Además, los análisis cefalométricos individuales o en serie han sido una técnica estándar utilizada por mucho tiempo para la evaluación del crecimiento.5, 18-20 Esta no es muy confiable, debido a la superposición de estructuras y la magnificación, por las diferentes distancias entre la película y los rayos x, además de que es una técnica 2D, por lo cual la interpretación de este examen tiene un valor limitado.9

Kubota, en 1999,21 reportó que el uso de trazados cefalométricos en las radiografías laterales no debe ser considerado un examen de rutina para asimetrías faciales, pues el tamaño del cóndilo va a ser medido solo en el lado derecho y el lado izquierdo no va a estar claro.

Radiografía panorámica: con la invención de la radiografía panorámica, en 1949, por Paatero, se han desarrollado con los años diferentes tipos de análisis, como el de Kjellberg y colaboradores22 y Levandoski,19 que nos permiten determinar en las radiografías panorámicas la simetría entre planos comparando ambos lados de la mandíbula en un mismo paciente, entregando una visión generalizada de la ar ticulación temporomandibular.21 Es una herramienta útil para evaluar las estructuras dentales y óseas del maxilar y mandíbula, pudiéndose detectar rápidamente asimetrías comparando tamaño, altura y forma de los cóndilos, cuerpo y rama mandibular. Su desventaja radica en la distorsión geométrica debido a las características de proyección que son inherentes al equipo con el que se toma la imagen, con lo que puede variar la forma de un área de la película a la otra.5-9

Radiografía posteroanterior: es una herramienta de gran valor para comparar las estructuras del lado derecho y el lado izquierdo. Esta radiografía es importante en casos de asimetrías, ya que, mediante el análisis de Grummons, se puede realizar un estudio comparativo de las alturas condilares, el tamaño de las ramas y cuerpos mandibulares, así como de la inclinación del plano oclusal, aunque también está limitada por la superposición de las estructuras.9, 22, 23

Tomografía computarizada: recientemente, esta técnica ha ganado interés en el diagnóstico de deformidades craneofaciales, gracias a las ventajas ofrecidas por el uso de software que permite mediciones precisas, mostrando los huesos craneofaciales desde diferentes ángulos por rotación de imágenes en tres dimensiones y observando el interior de las estructuras.24 La tomografía 3D es útil para evaluar con precisión las alteraciones morfológicas y estructurales de los huesos craneofaciales.25 Permite evaluar si el crecimiento óseo es generalizado o localizado en una parte de la cabeza del cóndilo.13 Igualmente, en la planificación quirúrgica juega un rol importante, ya que las predicciones basadas en cefalometrías tradicionales de dos dimensiones pueden ser muy difíciles e inexactas. Con un diagnóstico y planificación basada en tomografía de tres dimensiones, pueden verificarse los efectos de la corrección quirúrgica simulada en un modelo tridimensional.26

IMÁGENES DE MEDICINA NUCLEAR

Los procedimientos por imágenes de medicina nuclear son poco invasivos y, con la excepción de las inyecciones intravenosas para aplicar el radiofármaco, generalmente constituyen exámenes médicos indoloros que ayudan a los médicos a diagnosticar y evaluar problemas de salud. Estas exploraciones por imágenes utilizan materiales radioactivos denominados radiofármacos o radiotrazadores.

Para el diagnóstico de HC existen dos tipos de pruebas gammagráficas, con el fin de corroborar el diagnostico presuntivo, como son la gammagrafía ósea planar y la Tomografía Computarizada por Emisión individual de Fotones (SPECT).27-29

Gammagrafía ósea planar: (figura 1) es un examen que detecta áreas de aumento o disminución del metabolismo óseo. El examen se realiza para identificar procesos anormales que comprometen el hueso, tales como alteraciones en el crecimiento, tumores, infección o fracturas.7, 9

Kaban y colaboradores,30 en 1982, fueron los primeros en usar la gammagrafía ósea en un estudio de pacientes no asimétricos para determinar el grado de absorción del radiofármaco, de acuerdo a la edad y la maduración esquelética. La muestra consistía en 34 pacientes que oscilaban entre los 15 meses y 22 años de edad, a los cuales se les administraba Tc-99m MDP con el fin de determinar estándares de absorción del radiofármaco, encontrándose que a medida que aumenta la edad, disminuye en un patrón lineal la absorción y su tasa se acerca a valores por debajo de 1 cuando cesa el crecimiento. Los valores hallados por Cisneros y Kaban,31 en 1984, están representados en la tabla 1.

Para obtener la tasa, se realizó la siguiente operación:

La radiación de fondo es la que la gamacámara detecta proveniente del resto del organismo, y debe separarse de la emitida por los órganos blanco del trazador, lo cual es realizado por un computador. Los resultados se comparan con la tasa de captación del radionucleótido en cóndilos de pacientes normales.

Adicionalmente, en este estudio de pacientes simétricos no hubo diferencias de absorción entre los cóndilos por encima de 10%, por lo tanto, diferencias inferiores a este porcentaje de absorción se consideran normales. Posteriormente, realizaron el estudio con pacientes con asimetría mandibular (Cisneros, 1984), encontrando que la gammagrafía ósea cuantifica la captación en hueso del radiofármaco 99mTc-MDP en el cóndilo, la rama y regiones del cuerpo de la mandíbula.31

El tecnecio 99 es un emisor gamma que es activamente captado por el hueso cuando se inyecta por vía intravenosa unido a difosfonato de metileno (MDP), es metabolizado por las células óseas y es incorporado en las áreas donde el hueso nuevo se está formando o reabsorbiendo. Se utiliza para el diagnóstico con sospecha de HC unilateral o bilateral, su distribución se puede medir por una cámara de rayos gamma (gamacámara). La vida media del isótopo es de seis horas y está disponible comercialmente. La dosis de radiación es comparable a la de una radiografía del esqueleto del cuerpo entero de un niño, y la de una radiografía de cráneo normal seriada en adultos. La captación de tecnecio 99 MDP dentro del hueso es un reflejo de una actividad metabólica reciente. Es por esto que la gammagrafía, con este radiofármaco, puede servir como un buen método para la evaluación del crecimiento esquelético.30, 31

La gammagrafía ósea es un método instantáneo de comparación de la actividad metabólica celular entre el cóndilo normal y anormal. Puede utilizarse para distinguir el lado normal del anormal y para proporcionar información sobre la estabilidad o actividad de la hiperplasia, ya que es frecuente encontrar en la hiperplasia picos de exacerbación y mayor crecimiento, y de inactividad que solo es posible valorar mediante una gammagrafía.27

La gammagrafía es inespecífica , pues un resultado positivo puede producirse por cualquier clase de aumento en el metabolismo óseo, ya sea de carácter infeccioso, inflamatorio, traumático o neoplásico y aún en procesos de crecimiento normal.32

La gammagrafía ósea tiene 3 fases. La primera es la fase angiográfica, inmediatamente después de inyectar el radiofármaco o radiotrazador, donde se detecta el nivel de vascularización de la zona por medio de imágenes rápidas (2 a 5 seg/imagen). La segunda es la fase tisular, tomada aproximadamente a los 10 min, en la cual se evalúan los tejidos blandos adyacentes. Y la tercera es la fase ósea o tardía, que se toma durante las primeras cuatro horas.5

El SPECT es una técnica recientemente desarrollada para recoger y desplegar imágenes seriadas por planos paralelos de medicina nuclear (figura 2). Es una variación de la gammagrafía y consiste en un método de imagenología funcional, que refleja la respuesta metabólica del órgano en evaluación mediante el uso del radiofármaco (Tecnecio 99 MDP), el cual es llevado a un área determinada y controlada por el nivel de actividad metabólica e irrigación sanguínea de la región.28, 33, 34

El procedimiento implica la utilización de una cámara (gamacámara) que rota alrededor del paciente, generando proyecciones circunferenciales. Los cor tes de la tomografía axial, de aproximadamente 6,5 mm entre uno y otro, son computarizados de la misma manera que se hace con un tomógrafo de escáner de rayos X.29

SPECT se ha convertido en una herramienta de diagnóstico esencial para la visualización de la hiperactividad del cóndilo.6 Esta proporciona un contraste de imagen mejorada y una capacidad cuantitativa que podría permitir al clínico hacer una evaluación más exacta de la etapa de la actividad normal y asimétrica del crecimiento mandibular.35 El aumento de la absorción de los radionucleótidos por el cóndilo hiperplásico, puede ser indicio de la continuidad del crecimiento anormal.31, 36

Estas dos técnicas (SPECT y Gammagrafía planar), pueden ser analizadas de manera cualitativa (subjetiva) mediante la observación de las imágenes que entrega la gammagrafía, y cuantitativa (objetiva) mediante la cuantificación de la tasa de absorción de nucleótidos a nivel condilar.28

Saridin y colaboradores,28 en 2011, mostraron que la técnica SPECT para la detección de HC Unilateral es significativamente más sensible que la técnica de gammagrafía de hueso planar, con un 19% adicional de la técnica SPECT en la identificación de HC Unilateral.

Con el uso de la tecnología SPECT es posible obtener una imagen de ambos cóndilos y del clivus en el mismo corte. El clivus, por corresponder a la unión del hueso occipital con el cuerpo del esfenoides, se localiza en la base del cráneo, al mismo nivel axial de los cóndilos mandibulares.37

Se ha documentado que el clivus de la base del cráneo cesa su crecimiento cuando la sincondrosis esfenooccipital se fusiona. Una vez ocurre esta fusión, puede ser uno de los huesos menos metabólicamente activos en el cuerpo porque no tiene articulaciones y no soporta peso. Esta estructura, al igual que la vértebra lumbar 4 (L4), por sus características de mantenerse estables durante el crecimiento, han sido usadas como punto de referencia en estos métodos diagnósticos para establecer comparación cuantitativa de hipercaptación celular ósea entre ambos cóndilos.29, 38, 39

Las imágenes SPECT de pacientes con enfermedad de la base del cráneo, son más sensibles que las imágenes planares y pueden dar la ubicación anatómica de la patología y ser útiles en pacientes con una tomografía computarizada negativa.

Pogrel y colaboradores,29 en 1995, diseñaron un estudio para comparar dos métodos de cuantificación para la captación de tecnecio 99- MDP en el cóndilo mandibular, utilizando dos técnicas: una estándar de gammagrafía planar, teniendo como referencia la cuar ta vértebra lumbar (L4), y una técnica SPECT, la cual cuantifica la actividad del cóndilo comparada con la absorción en el clivus.

Encontraron que con la técnica SPECT se realizaba una mejor reproducibilidad que con la técnica de gammagrafía planar, pues la actividad en el clivus mostraba una menor variación de la actividad que la que presentaba la cuarta vértebra lumbar. Concluyeron que el uso de SPECT con el clivus como punto de referencia, resuelve el problema de la superimposición, que generalmente se genera con la gammagrafía planar y se evitan posibles problemas asociados con la absorción de L4.

La gammagrafía ósea Planar ha demostrado ser una prueba diagnóstica específica en un (85%) y sensible en un (67%),29 comparada con 93% de sensibilidad y 96% de especificidad de la prueba diagnóstica SPECT,40 con lo que podríamos acercarnos a un diagnóstico adecuado y brindar, por lo tanto, un tratamiento ideal, a tiempo, sin permitir que el paciente genere asimetrías faciales severas y así mejorar el pronóstico de la enfermedad.

La evaluación cuantitativa de la absorción del radiofármaco a nivel condilar, en casos de sospecha de Hiperplasia, puede realizarse detectando el porcentaje de captación entre los dos cóndilos comparativamente y, como se explicó anteriormente para identificar el Ratio, se relaciona la captación de las dos articulaciones con la cuarta vér tebra lumbar (L4) o el clivus, que, como se sabe, por su fácil ubicación y por su captación simétrica pueden servir de referencia.36, 40

Se ha documentado que una diferencia en la absorción de 10% entre ambos cóndilos, puede ser indicativo de asimetría, y que valores de absorción por encima del 55% son indicativos de HC.31, 32

DIAGNÓSTICO DIFERENCIAL DE LA HC

La HC unilateral debe ser diferenciada de otros procesos que causan asimetría facial, como la microsomía hemifacial, atrofia hemifacial, anquilosis y tumores óseos, macrognatismo unilateral (hipertrofia mandibular unilateral, que es la ampliación de la mandíbula en tres dimensiones, incluyendo cóndilo, rama y termina en la línea media de la sínfisis) y laterognatia (prognatismo asimétrico donde ambos cóndilos son de igual tamaño).13, 41

La microsomía hemifacial, clínicamente, se asemeja al patrón de crecimiento horizontal de la HC, aunque se debe a una hipoplasia condilar en el lado opuesto y, obviamente, no está acompañada de una hiperactividad en la región condilar. Se pueden diferenciar también por las numerosas características faciales y del oído que presenta la microsomía.42, 43

En el diagnóstico clínico diferencial también están las condiciones neoplásicas, tales como osteocondroma, osteoma, osteblastoma y condrosarcoma, que pueden mostrar características clínicas, radiográficas y oclusales similares a la HC.

Se diferencia del osteocondroma, pues en este el alargamiento está limitado al tumor. Las medidas de ambas mitades mandibulares son iguales, el tumor tiene márgenes aplanados, la cubierta de cartílago no es observada en la radiografía, puede ser observado como un crecimiento separado del hueso inicial, el crecimiento no es autolimitante. Además, puede ocurriren unespacio no disponible y crear asimetría mandibular y cambios en la oclusión.4

Para obtener una respuesta inmediata de la sospecha, pueden ser usadas técnicas tales como la gammagrafía o su variación SPECT.44

TRATAMIENTO DE LA HC

El tratamiento quirúrgico varía de acuerdo a la actividad de la enfermedad. La cirugía debe estar basada en los hallazgos de la captación de tecnecio 99MDP, que se usa para monitorear la actividad del crecimiento del cóndilo, ya que la radiolucidez está concentrada en áreas de aumento de actividad osteoblástica.14

El tratamiento de la deformidad mandibular es principalmente quirúrgico, consta de la intervención en función de la actividad del cóndilo: La condilectomía alta o afeitado del cóndilo del lado afectado, está indicada para limitar la progresión de la asimetría durante la fase activa del crecimiento excesivo condilar. Esta consiste en extraer la parte superior del cóndilo con una disección de 3-5 mm de la cabeza condilar, incluyendo los polos medial y lateral. Previamente, debe realizarse una disección profunda de la capa de la fascia y debe dividirse el cóndilo suavemente desde el disco articular. La línea de osteotomía generalmente es caudal al borde de la cabeza del cóndilo, y se realiza con una fresa de fisura de 2mm de profundidad. Al finalizar el corte, debe realizarse la reinserción del músculo pterigoideo externo suturando el tendón a la nueva cabeza condilar. Cuando la condilectomía se realiza junto a una cirugía ortognática bi o mono maxilar, se debería realizar en el siguiente orden: osteotomía maxilar, condilectomía y osteotomía mandibular. Por último, se debe garantizar que el cóndilo llegue a la fosa9 y se suturan la cápsula, la fascia y los ligamentos.45-47

La condilectomía debe hacerse una vez sea diagnosticada la HC, seguida de un tratamiento ortodóncico, ya sea de descompensación dentoalveolar, con el fin de preparar al paciente para una cirugía ortognática que corrija las secuelas de la enfermedad (asimetría), o de un tratamiento ortodóntico compensador, en casos en donde la enfermedad se haya detectado a tiempo y sea más un compromiso oclusal el que se deba corregir y no uno estético por alteración esquelética. Las consideraciones básicas en el manejo de la asimetría facial secundaria a HC, deben incluir el control del proceso de crecimiento para permitir un desarrollo facial equilibrado, así como el control del plano oclusal y la corrección de las compensaciones dentoalveolares que se producen durante la fase activa de la enfermedad.14, 16 En los casos donde en el diagnóstico se sospeche la presencia de un tumor, este procedimiento es obligatorio.13, 14, 39, 44

El tratamiento ortodóncico pre-quirúrgico de la hiperplasia en estado inactivo, es usado para alinear y nivelar los arcos dentales, para eliminar las compensaciones den-tales producidas en los tres planos del espacio y para corregir las inclinaciones, el torque y el eje axial de toda la dentición. Mientras que la ortodoncia post quirúrgica le entrega a los pacientes una oclusión ideal, funcional y estéticamente armónica con la nueva condición post quirúrgica del paciente.48

Wolford y colaboradores, en 2002,45 compararon los resultados del tratamiento de HC en un seguimiento a largo plazo de la estabilidad post-quirúrgica en pacientes diagnosticados con esta patología. Un grupo fue tratado solo con cirugía ortognática para corregir la asimetría, y el otro grupo fue tratado con condilectomía alta, reposicionamiento articular del disco y cirugía ortognática simultánea, demostrando que esta última es más estable y segura en el tiempo, con una diferencia estadísticamente significativa.

La condilectomía alta detiene el crecimiento excesivo y desproporcionado mandibular, mediante la extirpación quirúrgica de los principales sitios de crecimiento mandibular.49, 50

CONCLUSIONES

La literatura muestra que la HC se puede presentar a cualquier edad, pero hay predisposición en la etapa de crecimiento y desarrollo, además tiene prevalencia por el sexo femenino y la forma unilateral es más común que la bilateral.

De las tres formas de presentación de la HC, la Elongación Hemimandibular es la más común, caracterizándose por desviación del cuerpo mandibular y de los tejidos adyacentes hacia el lado no afectado.

El diagnóstico temprano de la HC permitiría establecer planes de tratamiento que frenen la patología en su fase activa y guíen el remanente de crecimiento en los pacientes, disminuyendo las secuelas que produce la patología cuando se diagnostica después de la etapa de desarrollo.

Es importante que el resultado de la gammagrafía sea presentado de manera cuantitativa, para poder conocer el porcentaje de absorción de cada cóndilo y, así, establecer la diferencia entre ambos. Teniendo en cuenta que valores de absorción por encima del 55% son indicativos de hiperplasia y diferencia de 10% entre ambos cóndilos indicaría asimetría.

Son necesarios estudios que establezcan el compor tamiento de la patología en nuestra población, que comparen las diferentes gammagrafías y que, a su vez, estos resultados se puedan correlacionar con el informe de patología, para así establecer con exactitud la especificidad de la prueba gammagráfica.

INTRODUCTION

Condylar Hyperplasia (CH) is a self-limiting pathologic condition that produces severe facial deformity at the expense of mandibular asymmetry. It is characterized by exaggerated and progressive growth of the condyle and can compromise the neck, the ramus, and the body of the mandible; it tends to be unilateral, although bilateral conditions also exist and can be accompanied by pain and articular dysfunction.1 It is commonly found in patients aged 11 to 25 years in its active form, and after this age it may be found in its passive form, usually like a clinical sequel left by the disease.1, 2 However, it can also occur in its active form in patients between the third and fifth decade of life, and even before the age of ten, and it has been reported to be more prevalent in women than in men.3 This condition can appear in three forms: Hemimandibular Hyperplasia (HH) which corresponds to asymmetry by vertical growth, Hemimandibular Elongation (HE), corresponding to asymmetry by horizontal growth, and a hybrid form of these two types.4

Its etiology is due to factors that may be genetic, traumatic, functional, hormonal, and tumor-related.4, 5

Diagnostic impression of CH can be made by a combination of clinical and radiological findings, next corroborated by means of nuclear medicine tests such as scintigraphy, and finally confirmed by pathology report when the patient undergoes surgery.

There are various diagnostic aids ranging from extraoral photographs, static and articulated study models, panoramic radiography, posterior-anterior radiography, tomography with 3D reconstruction, bone scan and SPECT-type scintigraphy, to histopathology studies for the diagnosis of hyperplasia.6

Early diagnosis is important since treatment modalities considerably differ depending on the affected structures, patient's age, severity of the asymmetry, and the active or passive status of the pathology.

It is necessary to treat the disease and to decide whether the center of growth is to be removed in cases in which active CH is detected or, on the contrary, to treat the sequel with orthognathic surgery and/or dentoalveolar orthodontic compensation when the pathology is inactive. This is why proper diagnosis of the pathology is important, and here is where TMJ bone scan should be essential for the clinician to obtain adequate information, ideally through quantitative data.7

DEFINITION

CH is a disorder characterized by excessive and progressive growth that affects the condyle's head and neck, as well as the mandible's body and ramus. The disease is self-limiting and deforming because growth is disproportionate even before the individual's overall growth has stopped, and can continue beyond its completion.4 This disease affects not only the patients' facial esthetics but also their masticatory functions. It is a condition that concerns not only maxillofacial surgeons but also orthodontists seeking to provide patients with normal occlusion, which is directly affected by an underlying pathology, like cell hyperactivity in the condyle.1

CLASSIFICATION

Obwegeser and Makek4 classified the asymmetry associated with CH in 3 categories: Hemimandibular Hyperplasia (HH), or asymmetry by vertical growth, Hemimandibular Elongation (HE), or asymmetry by horizontal growth, and a combination of these two types.

HH is characterized by a three-dimensional widening of one side of the mandible and excessive condyle growth, so that the condyle's neck looks thin and the ramus' height is bigger. It may produce ipsilateral open bite or over-eruption of maxillary teeth as they seek to achieve occlusion; there is very little deviation of the chin, and asymmetry occurs with decrease of the commissure of the affected side. There is compensation in the middle third by enlargement of the alveolar process, as well as downward inclination of the occlusal plane in the affected side, and even inclination in the orbital plane, but tooth midlines usually coincide.4

HE is the most common type of CH; it is characterized by normal size and shape of the condyle's head, but is accompanied by an elongated neck. There is chin deviation to the contralateral side, but there is no vertical increase of the ramus. The mandibular angle shows less alteration than in presence of vertical growth. Intraorally, the midline deviates towards the unaffected side, and occlusion usually shows contralateral cross bite, while the affected side displaces in mesial direction tending to Angle's Class III.4

The lower edge remains significantly unchanged and the compensatory changes of the middle third are mild, although there may be a slight inclination towards the floor of the orbit; it is also possible that both the philtrum of the upper lip and the nasal tip incline towards the unaffected side.

On the other hand, when the elongation is bilateral, it produces severe mandibular prognathism with or without asymmetry.

Type III is the hybrid form. It produces elongation and hyperplasia on the same side. The affected side is more voluminous, with its lower edge located below that of the opposite side. There is chin prominence with displacement towards the unaffected side and evident inclination of the occlusal plane.8

ETIOLOGY

The etiology of this disease is controversial; however, the following factors are the most relevant: genetic factors (congenital malformations, hemifacial microsomia, hemifacial atrophy), environmental factors (prenatal and postnatal trauma, infections, deficiencies in blood supply and hyper-vascularization, as well as neurotrophic disorders), functional factors (habits and occlusal interferences), hormonal factors (endocrine problems and growth factors), and neoplastic factors (osteoma, osteochondroma and chondroma).7, 9, 10

INCIDENCE

CH is usually unilateral and the literature reports a significant predisposition of women compared with men (2:1 ratio), with a prevalence of 57% in the right-side condyle compared to the left-side condyle, with 43%.3 In 2012, Raijmakers et al3 reported that female patients with unilateral CH outnumber male patients in the study populations, then being a female can be considered a risk factor for Unilateral CH (UCH).

Female prevalence has been associated with hormonal differences, especially with estrogens, since most CH patients reported in the literature have been found in the stage of development of secondary sexual characteristics-so it is possible that sex hormones are involved in the abnormal growth of the mandible-.3 The explanation may be that estrogens are one of the main regulators of bone growth and are expressed in articular cartilage and growth plates, since experimental evidence of local synthesis of estrogen has been found in the temporomandibular joint.10, 11

CHARACTERISTICS

It was first described by Robert Adams in 1836 6 as a condition that causes excessive development of the mandible and generates esthetic and functional alterations. Since then, there have been numerous reports in the literature referring to this clinical entity and the way it compromises facial esthetics and occlusal-functional alterations.

Characteristics of CH include uni- or bilateral enlarged mandibular condyle, so that the condyle's neck is enlarged out, inclining itself and showing downward growth below the mandible's body and ramus, causing enlargement of that side of the face and flattening of the contralateral side. Some patients may experience temporomandibular joint symptoms (TMD), such as pain, articular sounds, and limited mouth opening.12 Therefore, patients seek consultation due to marked facial asymmetry with mandibular deviation, malocclusion, and in some cases articular symptoms, usually noticed between the second and third decades of life.4, 12

It usually occurs in adolescence until the end of pubertal growth. If the deformity occurs before growth is completed, the occlusal plane usually inclines due to dental compensation, while posterior open bite may be evident if the deformity occurs once growth is completed.13

Some biomechanical phenomena may explain why deforming growth occurs in the mandible's body and ramus. Taking into account the mechanisms that maintain compression and tension forces within a range compatible with the tissue's health, in the presence of situations that produce loads exceeding the functional capacity of articular tissues, such as trauma, parafunction, or excessive and fast growth, the force vectors produce a spatial change of the system's structures, generating a sequence of events ranging from remodeling to degeneration and repair, resulting in forces redistribution. These articulatory changes are followed by morphological changes in the ipsilateral ramus and the mandibular body with impacts on facial harmony.4

HISTOPATHOLOGY OF CH

Hyperplasia is known as the increase in number of cells in a tissue, in this case causing excessive development of the condyle. Histologically, the active state of CH is characterized by the presence of a layer of undifferentiated mesenchymal cells and a layer of hypertrophic cartilage, along with islands of chondrocytes in trabecular bone, which is detected by technetium during scintigraphy-and calcification rate appears to be above normal ranges-.12 This cartilage is very thick and is replaced with new bone. This translates into hypermetabolism in the center of condylar growth on the affected side, with conversion of hyaline cartilage into fibrocartilage.2

According to Gray et al in 1990,12 there is another type of hyperplasia during the third and fourth decades of life, characterized by a delayed reaction of growth, in which histologically masses of hyaline cartilage are associated with degenerative areas of the TMJ and painful symptoms.6, 9 The center of growth, however, is located in the center of the condyle instead of the center of the fibrocartilage. When growth stops, the histological appearance is of a normal condyle with an irregular shape.1

Based on histopathology and clinical criteria, CH has been classified into two types: Active CH and Inactive CH (a stable situation in which abnormal growth is completed).14, 15 The active state shows a continuous articular surface and a large area of proliferation in which the hypertrophic cartilage layer shows average thickness. The inactive state is characterized by a discontinuous articular surface and a zone of proliferation in patches; also, the hypertrophic layer presents variable thickness and a large number of hypertrophic chondrocytes in some areas.16

DIAGNOSIS

Diagnosis of craniofacial asymmetries, one of them being CH, is important because treatment modalities considerably differ depending on the origin of the deformity, i.e., if it has a dental, skeletal, or functional origin. Several methods have been used to identify and quantify the severity of dental and facial asymmetries, including anamnesis, direct clinical evaluation, radiographic analysis, and evaluation of the face's vertical and horizontal proportions by means of facial photographs.7

Many diagnostic aids have been used to complement the clinical and physical examination of this pathology, including extraoral radiographs and photograph analysis, especially front views with a perfect position of the head to prevent alterations in the study of proportions. Study models are also of great help in the assessment of facial asymmetries, as they usually include a dental component.17

RADIOGRAPHIC METHODS FOR CH DIAGNOSIS

Conventional radiographs are used to determine the location of uni- or bilateral discrepancies that contribute to skeletal asymmetry, such as elongation of the mandibular condyle/neck of the involved condyle and the inclination of the mandible's body and ramus,13 and to assess the degree of deviation of dental and skeletal midlines from the mid-sagittal plane. Additionally, nuclear medicine offers planar bone scintigraphy and SPECT (Single-Photon Emission Computed Tomography), which allow a clear identification of active processes of condylar growth between inactive zones.

Lateral cephalic x-rays: it provides useful information about vertical asymmetries, sagittal projection of the asymmetry in ramus height, mandible length, and goniac angle. In addition, individual or serial cephalometric analyses have long been a standard technique for the assessment of growth.5, 18-20 The latter is not very reliable because of the superposition of structures and magnification, due to varying distances between the film and the x-rays; plus it is a 2D technique, so the interpretation of this test is limited.9

In 1999, Kubota reported that the use of cephalometric tracing on lateral radiographs should not be considered a routine test for facial asymmetries, since the size of the condyle will be measured on the right side only and there won't be clarity about the left side.21

Panoramic x-rays: Since the invention of panoramic radiography in 1949 by Paatero, different types of analysis have been developed over the years, such as the ones by Kjellberg et al22 and Levandoski,19 which allow using panoramic radiographs to determine symmetry between planes by comparing both sides of the mandible in the same patient, providing a general view of the temporomandibular joint.21 It is a useful tool to assess dental and bone structures of maxilla and mandible, as it enables a quick identification of asymmetries by comparing size, height and shape of the condyles, as well as the mandible's body and ramus. Its disadvantage lies in the geometric distortion due to projection features of the equipment used to take the images, so the shape of an area may vary from one film to the other.5-9

Posterior-anterior radiography: It is a powerful tool to compare the structures of the right side and left side. This type of radiography is important in cases of asymmetry because by means of Grummons analysis it allows performing a comparative analysis of condylar heights, as well as size of mandibular bodies and rami and the inclination of the occlusal plane, although it is also limited due to superimposition of structures.9, 22, 23

Computed Tomography: This technique has recently gained attention in the diagnosis of craniofacial deformities thanks to the advantages offered by the use of software that allows precise measurements, showing the craniofacial bones from different angles by rotating three-dimensional images and observing inside the structures.24 3D tomography is useful to accurately evaluate the morphological and structural alterations of craniofacial bones.25 It allows assessing whether bone growth is generalized or localized in one part of the condyle's head.13 Similarly, it plays an important role in planning surgery, since the predictions based on traditional two-dimensional cephalograms may be very difficult and inaccurate. With diagnosis and planning based on three-dimensional tomography, we can verify the effects of surgical correction in a three-dimensional model.26

NUCLEAR MEDICINE IMAGING

The imaging procedures of nuclear medicine are minimally invasive and, with the exception of intravenous injections to apply radiotracers, they usually are painless medical tests that help physicians diagnose and evaluate health problems. These scans by images use radioactive materials called radiopharmaceuticals or radiotracers.

There are two types of scintigraphic evidence to diagnose CH in order to corroborate the presumptive diagnosis: planar bone scintigraphy and Single-Photon Emission Computed Tomography (SPECT).27-29

Planar bone scintigraphy: (figure 1) This test detects areas of increased or decreased bone metabolism. The test is performed to identify abnormal processes compromising bones, such as growth alterations, tumors, infection, or fractures.7, 9

In 1982, Kaban et al30 first used bone scintigraphy in a study of non-symmetrical patients to determine the degree of absorption of the radiopharmaceutical according to age and skeletal maturation. The sample included 34 patients ranging from 15 months to 22 years of age, which were administered Tc-99m MDP in order to determine standards of radiopharmaceutical absorption, finding out that as age increases, absorption decreases in a linear pattern and its rate is close to values below 1 when growth stops. The values found by Cisneros and Kaban31 in 1984 are shown in table 1.

To obtain rate, the following operation was used:

Background radiation is the one the gamma camera detects as coming from the rest of the body, and it should be separated from the one emitted by the tracer's target organs-something that is performed by a computer-. The results are compared with the radionucleotide uptake rate in condyles of normal patients.

Additionally, in this study of symmetrical patients the difference in absorption between the condyles was no greater than 10%; therefore, differences below this percentage of absorption are considered normal. Subsequently, the researchers conducted a study on patients with mandibular asymmetry (Cisneros, 1984), finding out that bone scintigraphy quantifies the bone's uptake of radiopharmaceutical 99mTc-MDP in the condyle, the ramus, and regions of the mandible's body.31

Technetium 99 is a gamma emitter actively captured by bones when intravenously injected along with methylene diphosphonate (MDP); it is metabolized by bone cells and incorporated in areas where new bone is forming or resorbing. It is used to diagnose suspected unilateral or bilateral CH, and its distribution can be measured by gamma rays (gamma camera). The average lifespan of the isotope is six hours and is commercially available. The radiation dose is comparable to that of an x-ray of the skeleton of a child's entire body, or that of a normal skull x-ray series in adults. The uptake of technetium 99 MDP in bone is an indication of recent metabolic activity. For this reason, scintigraphy with this radiopharmaceutical can be used as a good method to evaluate skeletal growth.30, 31

Bone scintigraphy is an instant method of comparison of cellular metabolic activity between normal and abnormal condyles. It can be used to distinguish the normal side from the abnormal and to provide information about hyperplasia stability or activity, since it is often found that hyperplasia shows peaks of exacerbation and greater growth; it also provides information about inactivity, which is only possible to assess by means of scintigraphy.27

Scintigraphy is non-specific, since a positive result can be produced by any kind of increase in bone metabolism, either infectious, inflammatory, traumatic, or neoplastic in nature, and even in normal growth processes.32

Bone scintigraphy has 3 phases. The first is the angiographic phase, performed immediately after injecting the radiopharmaceutical or radiotracer; it detects the level of vascularization of the zone through fast images (2-5 sec/image). The second is the tissue phase, taken approximately 10 minutes later, which analyses adjacent soft tissues. And the third is the bone phase (or late phase), which is taken during the first four hours.5

SPECT is a newly developed technique used to collect and display serial images by parallel planes of nuclear medicine (figure 2). It is a variation of scintigraphy and is a method of functional imaging, which reflects the metabolic response of the organ under evaluation through the use of a radiopharmaceutical (technetium 99 MDP), which is brought to an area determined and controlled by the level of metabolic activity and blood flow in the region.28, 33, 34

The procedure involves the use of a gamma camera that rotates around the patient generating circumferential projections. Sections of the axial tomography, of approximately 6.5 mm between one and the other, are computerized in the same way as with an x-ray scanner tomograph.29

SPECT has become an essential diagnostic tool to observe condyle's hyperactivity.6 It provides enhanced image contrast and a quantitative capacity that enables the clinician to make a more accurate assessment of the stage of normal and asymmetric activity of mandibular growth.35 The increase in radionuclide uptake by the hyperplastic condyle may mean that abnormal growth continues.31, 36

These two techniques (planar scintigraphy and SPECT) can be analyzed in a qualitative (subjective) manner by observing the images delivered by scintigraphy, and in a quantitative (objective) manner by quantifying the condyle's rate of nucleotide absorption.28

In 2011, Saridin et al28 showed that the SPECT technique to detect Unilateral CH is significantly more sensitive than the technique of planar bone scintigraphy, with an additional 19% of the SPECT technique in identifying Unilateral CH.

The use of SPECT technology makes it possible to obtain an image of both condyles and the clivus in the same section. Since it corresponds to the intersection of the occipital bone and the body of the sphenoid, the clivus is located at the cranial base, at the same axial level of mandibular condyles.37

It has been shown that the clivus of the cranial base stops growing when the spheno-basilar synchondrosis fuses. Once this fusion occurs, it may become one of the least metabolically active bones in the body because it has no joints and does not support weight. As they remain stable during growth, this structure and the lumbar vertebra 4 (L4) have been used as points of reference in these diagnostic methods to establish quantitative comparisons of bone cell hyperperfusion between both condyles.29, 38, 39

SPECT images of patients with disease of the cranial base are more sensitive than the planar images and can provide the anatomical location of the pathology and be helpful in patients with negative CT scan.

In 1995, Pogrel et al29 designed a study to compare two methods of quantification of technetium 99-MDP uptake in the mandibular condyle by means of two techniques: a standard planar scintigraphy, having the fourth lumbar vertebra (L4) as a reference, and a SPECT technique, which quantifies the condyle activity compared with absorption in the clivus.

They found out that SPECT provided better reproducibility compared to planar scan, since the activityoftheclivusshowedalowervariationthanthat of the fourth lumbar vertebra. They concluded that using SPECT with the clivus as a point of reference solves the problem of superimposition, which usually generated by planar scan, and prevents possible problems associated with the absorption of L4.

Planar bone scintigraphy has proven to be a specific diagnostic test in (85%) and a sensitive test in (67%),29 compared with 93% sensitivity and 96% specificity of the SPECT diagnostic test,40 which could bring us closer to proper diagnosis and therefore provide an ideal timely treatment, preventing severe facial asymmetries in the patient and improving the prognosis of the disease.

The quantitative evaluation of radiopharmaceutical absorption at the condylar level in cases of suspicion of hyperplasia can be done by comparatively detecting the percentage of uptake between the two condyles, and as explained above to identify the rate, uptake of both joints is related to the fourth lumbar vertebra (L4) or the clivus, which, as it is well known, can serve as a reference due to its easy location and symmetrical uptake.36, 40

It has been shown that a 10% difference in absorption between both condyles may indicate asymmetry, and absorption values above 55% are an indication of HC.31, 32

DIFFERENTIAL DIAGNOSIS OF CH

Unilateral CH should be distinguished from other processes that cause facial asymmetry, such as hemifacial microsomia, hemifacial atrophy, ankylosis and bone tumors, unilateral macrognatism (mandibular unilateral hypertrophy, or the enlargement of the mandible in three dimensions, including condyle, ramus and midline of the symphysis), and laterognatia (asymmetric prognathism in which both condyles are of equal size).13, 41

Clinically, hemifacial microsomia resembles the pattern of horizontal growth of CH, although it is due to condylar hypoplasia in the opposite side and obviously it is not accompanied by hyperactivity in the condylar region. They can also be distinguished by the numerous facial and ear features produced by microsomia.42, 43

The differential diagnosis also includes neoplastic conditions, such as osteochondroma, osteoma, osteblastoma and chondrosarcoma, which may show clinical, radiographic, and occlusal characteristics similar to those of CH.

It differs from osteochondroma because in this disease enlargement is limited to the tumor. The dimensions of both mandible halves are equal, the tumor has flattened margins, cartilage covering is not observed in the x-ray, it can be seen as a separate initial bone growth, and growth is not selflimiting. Also, it can occur in a non-available space and create mandibular asymmetry and changes in occlusion.4

For an immediate response upon suspicion, techniques such as scintigraphy or its SPECT variation can be used. 44

TREATMENT OF CH

Surgical treatment varies according to the level of activity of the disease. Surgery should be based on the findings of technetium 99 MDP uptake, which is used to monitor the activity of condyle growth, since the radiolucency is concentrated in areas of increased osteoblastic activity.14

Treatment of mandibular deformity is mainly surgical; it consists of intervention based on the activity of the condyle: high condylectomy or trimming of the condyle on the affected side is intended to limit the progression of the asymmetry during the active phase of condylar overgrowth. It consists of removing the top of the condyle with a dissection of 3-5 mm of the condylar head, including the medial and lateral poles. This must be preceded by a deep dissection of the fascia layer and the condyle must be gently divided from the joint disk. The osteotomy line is usually caudal to the edge of the condyle head, and is performed with a 2 mm deep fissure but. Once the section has been made, reintegration of the pterygoid external muscle must be achieved by suturing the tendon to the new condylar head. When condylectomy is performed along with bi- or mono-maxillary orthognathic surgery, it should be done in the following order: maxillary osteotomy, condylectomy, and mandibular osteotomy. Finally, the clinician must make sure that the condyle reaches the fossa9 and to suture the capsule, the fascia and the ligaments. 45-47

Condylectomy must be performed once CH has been diagnosed, followed by orthodontic treatment either by dentoalveolar decompensation, in order to prepare the patient for orthognathic surgery to correct sequels of the disease (asymmetry), or by compensatory orthodontic treatment in cases where the disease has been detected in time and what needs to be corrected is an occlusal compromise instead of an esthetic condition by skeletal alteration. The basic considerations in the management of secondary facial asymmetry or CH should include control of the growth process to allow for balanced facial development as well as control of the occlusal plane and correction of the dentoalveolar compensations that occur during the active phase of the disease.14, 16 In cases where the diagnosis includes suspicion of the presence of tumor, this procedure is compulsory.13, 14, 39, 44

Pre-surgical orthodontic treatment of hyperplasia in its inactive state is used to align and level the dental arches in order to eliminate the dental compensation produced in the three spatial planes and to correct inclinations, torque and axial shaft of the entire dentition, while post-surgical orthodontics provides patients with and ideal occlusion which is at the same time functional and esthetically harmonious with the new post-surgery patient condition.48

In 2002, Wolford et al45 compared the results of CH treatment in a long term follow-up of postoperative stability in patients diagnosed with this disease. One group was treated with orthognathic surgery only to correct asymmetry, while the other group was treated with high condylectomy, articular disc repositioning and simultaneous orthognathic surgery, proving that the latter is more stable and reliable over time, with a statistically significant difference.

High condylectomy stops excessive and disproportionate mandibular growth by surgical removal of the main sites of mandibular growth.49, 50

CONCLUSIONS

The literature shows that CH can occur at any age, but there is predisposition during the stage of growth and development; also, it is prevalent among females and the unilateral form is more common than the bilateral.

Of the three forms of manifestation of CH, Hemimandibular Elongation is the most common, characterized by deviation of the mandible body and adjacent tissues towards the unaffected side.

Early diagnosis of CH allows for the establishment of treatment plans that stop the pathology in its active phase and guide the remnants of patients' growth, reducing the sequels left by the disease when diagnosed after the stage of development.

It is important to present the scintigraphy results in a quantitative manner in order to know the percentage of absorption of each condyle and thus establish the difference between the two. Taking into account that absorption values over 55% are indicative of hyperplasia and a 10% difference between both condyles suggest asymmetry.

It is necessary to perform studies that establish the behavior of the pathology in our population, comparing different scintigraphic scans and, in turn, these results should be correlated with pathology reports in order to exactly establish the specificity of scintigraphic tests.
