How do we decide which surgical technique is better? What are the parameters that should guide a surgeon, in the quest for the perfect surgical solution, to select one approach over the other?
We strive to make our choices scientic, but we nd it a difcult task to accomplish: we are overwhelmed by multiple studies that many times provide statistical evidence for contradicting conclusions, and differ by their methodology, power, and biases. The battle between laparoscopic and open hernia repair is a good example: despite obvious attempts to be neutral and objective, proponent and opponents may come up with different conclusions.
The study by Kouhia et al. in this issue [1], is important, as it gives us a viewpoint of a large-scale, unselected, population-based study and not the size-limited, selected groups characteristic of prospective RCTs. So, in a way, this methodology...
