Introduction: There are many congenital anomalies affecting the respiratory system. Some of these anomalies may cause stillbirth, some of which may lead to severe respiratory distress in newborns, some of them may remain asymptomatic for a long time.
Case: 25 years old, gravida1, parity 0, 22 weeks pregnant patient is applied to our clinic due to hydrops fetalis. First trimester screening was normal, maternal blood type was A Rh(+), indirect Coombs test was negative, TORCH screening was negative and there was no additional maternal disease. In the evaluation at our perinatology clinic isolated fetal ascites is detected, MCA PSV was 1,07mom, and fetal biometry was compatible with 23-24 weeks. Proximal trachea was atretic and bilateral fetal lungs were hyperechogenic. Chordocentesis is performed due to fetal ascites and fetal karyotype was normal. The patient is discussed at our council and the option for termination of pregnancy is given to the patient. Termination of pregnancy is performed due to the request of patient.
Conclusion: Fetal high airway obstruction should be kept in mind for the patients with hydrops findings in collabration with fetal hyperechogenic lungs.
