How do you secure your mesh when you repair a hernia? It all depends you would probably answer-justiably so. The number of variations and combination seems to be endless:
Interrupted versus continuous suture?
Absorbable versus non-absorbable? Trans-abdominal?
Tacks?
Biologic glue? Synthetic glue? Self-adhering mesh?
Surely it depends on many factors: type of the hernia, size of the defect, location of the mesh, and surgical technique. It also depends on local habits and dogmas, personal preferences, and beliefs. And we should not forget surgical fashion and industry pressurethe number of xation devices and special self-adhering meshes is increasing exponentially.
A few parameters can be measured to prove superiority of one method over the other, such as recurrence rate, chronic pain, and foreign body sensation, and then we have the minor factors such as cost and availability.
