"ACCOUNTABILITY" E REDUÇÃO DAS DESIGUALDADES EM SAÚDE: A experiência de São Paulo 1

Abstract

Em São Paulo, as desigualdades em saúde decresceram entre 2001 e 2016. Esse fato não é trivial, dada a bem conhecida dificuldade em reverter desigualdades. A partir do conceito de accountability, fundado na teoria de principal-agente, enfoca-se a implementação da política de atenção primária à saúde no município. A análise destaca o papel da acirrada competição eleitoral, dos conselhos de saúde e dos contratos - entre o governo federal e o município, e entre este último e as oss - para promover esses resultados.

In the megacity of São Paulo health inequalities decreased between 2001 and 2016. These distributive results are striking given how difficult it is to reverse inequalities. The article focuses on the implementation of the primary health care policy in the city, highlighting the role of three accountability mechanisms in promoting these results: fierce electoral competition, health councils and contracts - between the federal government and the municipality, and between the latter and the oss.
