A 94-year-old man was receiving prophylactic dosulepin 175mg for depression [route of administration, frequency and duration of treatment not stated]. He was admitted to hospital following frequent falls, and was found to be dehydrated with a urinary tract infection. Orthostatic hypotension was attributed to dosulepin use, and the drug was discontinued. In the following 72 hours, he deteriorated and developed delirium. It was suspected that the delirium was a result of dosulepin withdrawal, and he was re-prescribed dosulepin. His symptoms resolved swiftly.Author Comment"It has been hypothesized that antidepressant-induced supersensitivity of central and peripheral muscarinic cholinergic mechanisms may account for commonly observed antidepressant withdrawal phenomena."
