With a healthcare ecosystem approach, designated General Medicine physicians conducted post-discharge house visits to patients at risk of frequent readmissions (Hospital-to-Home Programme) during the study period, and provided medical support to institutionalised patients in the community, such as those at Institute of Mental Health, destitute and nursing homes within the precinct through a “hub and spoke” approach. Data collection and analysis plan To evaluate the clinical efficacy of its service implementation, de-identified quantitative demographic and clinical data including length of stay, 30-day readmissions, inpatient mortality and gross hospitalisation bills were extracted from hospital information system for General Medicine patients admitted between October 2018 and March 2019.
