An 81-year-old man developed a petechial purpuric rash and possible leucocytoclastic (hypersensitivity) vasculitis during antibiotic therapy with cefazolin [duration of treatment to reactions onsets not stated].
The man presented to hospital with shortness of breath, nausea, vomiting, fever, increasing pain, redness and swelling of his left knee for 2 days. He had undergone left knee arthroplasty in 2000, followed by explantation in February 2017 due to a methicillin-sensitive Staphylococcus aureus (MSSA) infection, with re-implantation in early 2018. In October 2018, he underwent incision and drainage due to the infection; MSSA growth was noted in drainage cultures. His spouse stated that he was experiencing decreased urine output, as well as bilateral petechial rashes on his lower limbs, which extended to his thighs. His medical history was notable for IgA...
