A retrospective study described 8 men, aged 60-82 years, who developed grade 3-4 neutropenia and/or grade 3 infection following the administration of cyclophosphamide [duration of treatment to reaction onsets and outcomes not stated].
Patient 1: A 60-year-old man, who had a history of diffuse large B cell lymphoma and haemodialysis three times a week, received IV cyclophosphamide at 750 mg/m2 as a part of the first cycle of R-CHOP regimen which also included rituximab, doxorubicin, vincristine and prednisolone; repeated every three weeks. The cyclophosphamide dose for the subsequent cycles was adjusted based on the attending physician's opinion. Subsequently, he developed grade 3 neutropenia and grade 3 infection as a consequence of impaired cyclophosphamide clearance. He received six cycles of the R- CHOP regimen with a complete response.
Patient 2: A 72-year-old man, who had a history of diffuse large B cell lymphoma and haemodialysis three times a week, received IV cyclophosphamide at 70% of 750 mg/m2 as a part of the first cycle of R-CHOP regimen which also included rituximab, doxorubicin, vincristine and prednisolone; repeated every three weeks. The cyclophosphamide dose for the subsequent cycles was adjusted based on the attending physician's opinion.
