A 48-year-old woman developed antimuscarinic delirium secondary to drug toxicity following diphenhydramine overdose in suicide attempt.
The woman presented after ingesting diphenhydramine 3.75g (41.2 mg/kg) in an attempt to commit suicide. She had a history of depression. On presentation, she was somnolent, hypoxic, vomiting and had slurred speech. She received supplemental oxygen for presumed aspiration. Her home medications included citalopram, bupropion, clonidine, clonazepam, lisdexamfetamine, modafinil, lurasidone, oxycodone, oxybutynin and trazodone. On presentation, her body temperature was 36.8°C, HR was 124 bpm, BP of 1 71/112 mmHg, RRof 37, and oxygen saturation of 94% on 6-L nasal cannula. EKG revealed sinus tachycardia....
