"One of the things we know for sure is that nobody was ever trying to get hurt," says Safestart's Vice President Larry Wilson about workplace injuries. "With the exception of contact sports and military combat, no one was ever intentionally trying to hurt anyone else either," Wilson says.
Yet, the most common sources of unexpected events are self, people and equipment, he says. Either a person's own actions contribute to an incident, other peoples' behaviors cause accidents or something unexpected happens with equipment, such as a wire rope breaking or a hose bursting.
According to Wilson, four critical errors are common among incidents.
