With targeted MURs launching in England in three weeks, Trudy Thomas looks at counselling patients discharged from hospital
Patients who have recently been discharged from hospital and have had changes made to their medicines during their stay are a target group for MURs.
The MUR should ideally take place within four weeks of the patient being discharged, but may take place up to eight weeks after the discharge date.
The MUR should focus on the medicines that have been stopped, changed and added. There is often significant confusion post-discharge for patients, GPs and pharmacists, and the MUR is an ideal opportunity to clarify which medicines are being continued or stopped, and why. However, it is important the patient is given the chance to ask any questions about any of their medicines throughout the MUR.
